RAAFA Connect (formerly known as AFA Care) is the in-home support division of RAAFA, one of WA's leading providers of tailored, customer focused supported living services.  Our aim is to provide you with the support you need in the comfort and familiarity of your won home, optimising your independence and safety.

Our nurses and support staff work together to imrpove your ability to stay at home.  We achieve this by listining to your needs and designing a personalised service that specifically suits your requirements and budget.

The team at Connect understands the utmost improtance of providing high quality care to our clients.  We strive to provide you with consistent, well qualified staff with the right skills to complete the tasks you require.


We can support you in the following ways:

Help around your home - This includes assistance with housework such as laundry, cleaning and cooking.

Help with yoru personal care - This includes showering, assitance with medication and grooming.

Help to get out and about - This includes shopping, going to appointments and social activities.

Help with your health - This includes nursing, physiotherapy, health reviews, equipment to maintain yoru independence, exercise and help to manage chronic disease.


RAAFA has a long standing involvement with both veterans and the community alike and is authorised to deliver Community Nursing services to entitled veterans.
